Membership Conditions

1. Any Associate member who wishes to resign from the Club shall give seven days notice, in writing, to the Administration office.

2. Any associate member who resigns will not be eligible for re-admission for a period of two years unless good and sufficient reasons are given and accepted by the Club Committee..

3. Any application for Junior membership must have parental consent and an undertaking, by the parent, to make good any damage or injury caused by the junior member.

4. I agree that the GlaxoSmithKline Sports & Social Club shall not be liable for the disappearance, loss or theft of, or damage to, any of my personal property including, but not limited to, any money, negotiable securities, jewellery, furs. I also understand parking my vehicle is done so at my own risk.
A copy of the Club rules and regulations (The Constitution) is available from the Club Administration office on request.
I agree to abide by the rules and regulations of GlaxoSmithKline Sports and Social Club. By joining, I agree to the club processing and storing my details for seven years and in IT form for as long as I am a member. I also understand it is my right to request access to this information at any time and to request the appropriate erasure of my details whenever I no longer wish to have them retained by the club.

Should I wish to participate in an Exercise Programme (“Programme”) offered at the GlaxoSmithKline Sports & Social Club Fitness Suite (“Facility”). I understand that these activities may include gymnasium, squash and group exercise (aerobic and resistance). In return for the GlaxoSmithKline Sports & Social Club Fitness Suite accepting me as a participant in the Programme, I represent and agree as follows:

1. I understand the nature and the purpose of the Programme and am aware that any strenuous physical activity involves certain risks; I assume the risk of any and all accidents or injuries of any kind, which may be sustained by me by reason of, or in connection with, my participation in the Programme. I release, discharge and absolve GlaxoSmithKline Sports & Social Club and their officers, directors, employees and agents and each of their parents, affiliates, and subsidiaries from any and all liability or responsibility for any such accident or injury except to the extent such accident or injury is caused by or results from the negligence GlaxoSmithKline Sports & Social Club or any of their officers, directors, employees and/or agents. This release shall be binding upon my heirs, executors, administrators and assignees.

2. I agree to abide by all the rules and regulations of the Programme and facility and understand variations to these may occur from time to time. All information will be available by the club notice boards and the administration office.

3. I understand that I should not use any equipment that I am not confident in my abilities to use safely. I understand that on arrival to the fitness suite I will be required to complete a health history questionnaire.
